Because markquestion requested it, here’s my Pathfinder conversion of the mind flayer from Dungeons & Dragons 3rd Edition.
If this bothers anyone in a legal sense, let me know and I’ll take it down.
MIND FLAYER (Illithid)
LE Medium aberration
Init: +7; Senses: Darkvision 60 ft.; Perception +25
AC: 21, touch 13, flat-footed 18 (+3 Dex, +8 natural)
hp: 91 (14 HD)
Fort +6, Ref +7, Will +13
Speed: 30 ft.
Melee: 4 tentacles +14 (1d6+2 plus grab)
Special Attacks: Extract
Spell-Like Abilities (CL 9th, concentration +14):
At will - Charm monster (DC 19), detect thoughts (DC 17), levitate, mind blast (60 ft. cone, Will DC 19 negates, stunned for 3d4 rounds), plane shift, suggestion (DC 18)

Extract (Ex): A mind flayer that begins its turn with all four tentacles attached and that makes a successful grapple check automatically extracts the opponent’s brain, instantly killing that creature. This power is useless against constructs, elementals, oozes, plants, and undead. It is not instantly fatal to foes with multiple heads, such as ettins and hydras.
Grab (Ex): This ability follows the standard rules for the grab universal monster ability (Bestiary pg. 301). In addition, if a mind flayer begins its turn with at least one tentacle attached, it can try to attach its remaining tentacles with a single grapple attempt. The opponent can escape with a single successful grapple check or an Escape Artist check, but the mind flayer gets a +2 circumstance bonus for every tentacle that was attached at the beginning of the opponent’s turn.
Mind Blast (Sp): This is the equivalent of a 4th-level spell.
PSIONIC MIND FLAYERS
If you are using Psionics Unleashed from Dreamscarred Press, make the following changes to the mind flayer’s stats.
The mind flayer gains the (psionic) subtype.
Psi-Like Abilities (ML 9th, concentration +14): These replace the mind flayer’s spell-like abilities.
At will - Compelling voice (4 targets, DC 20), defy gravity, empathic connection (all types, 9 days, DC 20), mind blast (60 ft. cone, Will DC 19 negates, stunned for 3d4 rounds), read thoughts (DC 20), planar travel
Skills: Replace the mind flayer’s ranks in Knowledge (arcana) with ranks in Knowledge (psionics).
Mind Blast (Ps): This is the equivalent of a 4th-level power.
